PRACTICE POLICIES

Late arrival/Cancellation/No-show

If you’re a new client, welcome! Please plan to arrive 15 minutes before your scheduled session to fill out intake forms and to discuss session goals.

 

Returning clients, welcome! I’m dedicated to providing useful bodywork sessions that address your needs and concerns. Arriving on time for your scheduled appointment allows us to get to most out of our time together. Please arrive 5 minutes prior to your scheduled appointment to allow for discussing session goals and dress-down time. Clients who arrive late will be treated for the remainder of the scheduled session and will be charged for the full session.

 

Please notify me at least 24 hours in advance if you are unable to attend your appointment for any reason. After the first incident, any late cancellations will incur an automatic charge of 50% of the price of scheduled services. Allowances can be made at the discretion of the therapist. No-show incidents will be automatically charged for the full price of scheduled services.

Right of Refusal
I reserve the right to refuse service to anyone including and not limited to a client who arrives for a session under the influence of intoxicants, seeks treatment that is outside of my scope of practice, and anyone who makes sexual gestures. In the event of sexual gestures or harmful behaviors expressed by the client toward the therapist, the session will be stopped immediately, the client will be asked to leave, and the client/therapist relationship will be terminated indefinitely. I reserve the right to charge for the full amount of scheduled services under any of these circumstances regardless if treatment was rendered or not.

 

Sick Policy
If you are feeling unwell, let’s reschedule! If you are experiencing any of the following symptoms: sore throat, cough, diarrhea, vomiting, fever, contagious skin legions, conjunctivitis, head lice, hepatitis A, or other sudden illness - including COVID-19 symptoms within the last 14 days - please contact me as soon as possible and treatment will be postponed until you are feeling healthy.

​Payment Options
I accept card, cash, and gift certificates purchased from Julia Lynn LMT. Payment is due at the time of service.

CODE OF ETHICS

This Code of Ethics is a summary statement of the standards by which massage therapists agree to conduct their practices and is a declaration of the general principles of acceptable, ethical, professional behavior. Massage therapists shall:

  1. Demonstrate commitment to provide the highest quality massage therapy/bodywork to those who seek their professional service.

  2. Acknowledge the inherent worth and individuality of each person by not discriminating or behaving in any prejudicial manner with clients and/or colleagues.

  3. Demonstrate professional excellence through regular self-assessment of strengths, limitations, and effectiveness by continued education and training.

  4. Acknowledge the confidential nature of the professional relationship with clients and respect each client's right to privacy.

  5. Conduct all business and professional activities within their scope of practice, the law of the land, and project a professional image.

  6. Refrain from engaging in any sexual conduct or sexual activities involving their clients.

  7. Accept responsibility to do no harm to the physical, mental and emotional well-being of self, clients, and associates.

INFORMED CONSCENT TO MASSAGE THERAPY TREATMENT

  • I understand that the massage therapist is providing massage therapy services within their scope of practice as defined by the Oregon Board of Massage Therapists. 

 

  • I hereby consent for my therapist to treat me with massage therapy for the above noted purposes including such assessments, examinations, and techniques, which may be recommended, by my therapist.

 

  • I acknowledge that the therapist is not a physician and does not diagnose illness or disease or any other physical or mental disorder. I clearly understand that massage therapy is not a substitute for a medical examination. It is recommended that I attend my personal physician for any ailments that I may be experiencing. I acknowledge that no assurance or guarantee has been provided to me as to the results of the treatment. I acknowledge that with any treatment there can be risks and those risks have been explained to me and I assume those risks.

 

  • I acknowledge and understand that the therapist must be fully aware of my existing medical conditions. I have completed my medical history form as provided by my therapist and disclosed to the therapist all of those medical conditions affecting me. It is my responsibility to keep the massage therapist updated on my medical history.  The information I have provided is true and complete to the best of my knowledge.

 

  • I authorize my therapist to release or obtain information pertaining to my condition(s) and/or treatment to/from my other caregivers or third-party payers.

 

  • I have read the above noted consent and I have had the opportunity to question the contents and my therapy. I confirm my consent to treatment and intend this consent to cover the treatment discussed with me and such additional treatment as proposed by my therapist from time to time, to deal with my physical condition and for which I have sought treatment. I understand that at any time I may withdraw my consent and treatment will be stopped.
